St. Joseph/Cass Counties: Fostering Health Partnerships Meeting

The Michigan Chapter of the American Academy of Pediatrics (MIAAP), in partnership with the Michigan Department of Health and Human Services (MDHHS), will host a Learning Collaborative in your area. We will focus on concrete ways to improve health outcomes for children and youth in foster care.

Walk away with tools to:
• Improve coordination and continuity of care between Primary Care, MDHHS, mental health and oral health communities,
• Address barriers to care coordination and management for children in the foster care system in your county, and
• Better understand state policy requirements and its impact on you and how you do your job.
